Overview We are seeking a dedicated and compassionate Memory Care Coordinator to lead and oversee the specialized care of residents with Alzheimer's disease and other forms of dementia within our assisted living community. This vital role combines leadership, clinical expertise, and a passion for enhancing the quality of life for our residents. As a Memory Care Coordinator, you will develop personalized care plans, supervise care staff, ensure compliance with health regulations, and foster a supportive environment that promotes dignity, independence, and well-being for individuals living with memory impairments. This position offers an exciting opportunity to make a meaningful difference in the lives of seniors while advancing your career in long-term care management. Responsibilities Lead the development, implementation, and ongoing evaluation of individualized memory care plans tailored to each resident's needs and preferences. Supervise and mentor care staff, ensuring high standards of compassionate, person-centered care aligned with best practices in dementia and Alzheimer's care. Monitor residents' health status regularly, coordinate medical appointments, medication administration, and emergency responses as necessary. Ensure compliance with all state and federal regulations related to long-term care, assisted living standards, and dementia-specific policies. Collaborate with healthcare professionals, families, and community resources to support residents' physical, emotional, and social well-being. Manage documentation accurately using Point Click Care EMR tool for care records, incident reports, and regulatory compliance reports. Oversee staff scheduling, training programs, and performance evaluations to maintain a motivated and competent team dedicated to resident safety and comfort. Qualifications Proven experience in memory care administration or long-term care management with a focus on memory or dementia care. Extensive knowledge of Alzheimer's disease, dementia care strategies, resident rights, and long-term care regulations. Strong leadership skills with experience supervising care teams in assisted living or residential care settings. Background in nursing or healthcare with relevant certifications; nursing leadership experience highly preferred. Familiarity with Microsoft Office applications and Point Click Care EMR for documentation and reporting purposes. Knowledge of operational oversight within senior living facilities. Excellent communication skills to collaborate effectively with residents' families, staff members, healthcare providers, and regulatory agencies.
