Job Description
Clinical Care Coordinator - MA / EMT / Paramedic Take the Next Step in Your Clinical Career at Kymera. Kymera Independent Physicians is a physician-owned, multidisciplinary healthcare organization with clinics in Roswell, Carlsbad, and Hobbs , serving patients throughout Southeastern New Mexico. We are seeking an experienced Clinical Care Coordinator to support patient care across our multidisciplinary practice. This position is ideal for an experienced Medical Assistant, EMT, or Paramedic who has developed strong clinical knowledge, organizational skills, and independent judgment and is ready for a higher-level clinical support role. This is not a supervisory or management position . Instead, the Clinical Care Coordinator serves as an experienced clinical resource responsible for independently managing important patient-care workflows involving population health, specialty medication and injection tracking, prescription refill requests, and clinical patient portal communications. The successful candidate must be comfortable working independently, managing multiple priorities, communicating with providers and clinical teams, and supporting patients across multiple medical specialties. What You'll DoPopulation Health Management Audit and manage population health reports and patient worklists. Review patient records to identify outstanding preventive, chronic-care, and other population health needs. Identify patients who are due or overdue for applicable screenings, testing, follow-up care, or other services. Coordinate with providers, clinical staff, and scheduling personnel to address identified care needs. Initiate appropriate patient outreach and facilitate scheduling or follow-up as directed by established protocols. Track outstanding items through completion and maintain accurate documentation of actions taken. Assist with closing care gaps and maintaining accurate population health data. Identify discrepancies or concerns requiring provider or management review. Specialty Injection Tracking Manage tracking of patients receiving specialty injections and other designated medications. Maintain accurate records of scheduled and completed injections and identify patients requiring follow-up. Coordinate with providers, clinical staff, patients, pharmacies, and other parties as necessary to support continuity of treatment. Track applicable orders, medication availability, required testing, follow-up requirements, and other components of the specialty injection process. Identify delays, missing information, or other barriers that could interrupt treatment and coordinate appropriate follow-up. Maintain accurate documentation throughout the process. Prescription Refill Requests Review and process prescription refill requests in accordance with provider instructions, established protocols, and Company policy. Review the patient record for information necessary to process refill requests, including medication history, applicable laboratory testing, recent and upcoming appointments, and other required clinical information. Route requests requiring provider review or authorization to the appropriate provider. Identify requests that do not meet established refill criteria and coordinate appropriate follow-up. Communicate with patients, pharmacies, providers, and clinical staff as necessary. Document refill-related activity accurately and timely in the patient's medical record. Patient Portal Clinical Messaging Manage incoming clinical patient portal messages and associated clinical work queues. Review messages to determine the nature of the patient's request and the appropriate next step. Respond to messages within established protocols and scope of responsibility. Route clinical questions or concerns requiring provider review to the appropriate provider or clinical team. Recognize potentially urgent patient concerns and escalate them promptly in accordance with established procedures. Coordinate follow-up appointments, testing, prescription requests, or other services arising from patient communications. Ensure portal communications and follow-up actions are documented accurately and completed timely. A Higher-Level Clinical Role This position is designed for an experienced clinical professional who can do more than complete assigned tasks. The Clinical Care Coordinator must be able to manage a workflow from beginning to end , recognize when information is missing, determine the appropriate next step within established protocols, follow up on outstanding items, and know when an issue needs to be escalated to a provider or other appropriate personnel. You will work with multiple providers and specialties , requiring the ability to learn and apply different clinical workflows, protocols, and provider preferences while maintaining accuracy and consistency. When work is completed and coverage needs are met, employees may be released early to begin their weekend.
