Perform services in accordance with the physician's orders and the established plan of care (POC). Qualifications Education and Experience (ESSENTIAL): Must possess a bachelor's degree from a school of social work accredited by the council on social work education. Must be certified or licensed in the state where they currently practice. Must have at least one year of social work experience in a health care setting. Must have basic demostrated technology skills, including operation of a mobile device If employment with the hospice branch of Enhabit Home Health & Hospice began prior toDecember 2, 2008, may not require supervision by a licensed master of social work. Education and Experience (DESIRED): May have a bachelor's degree in psychology, sociology, or other field related to social work; must becertified or licensed in the state where they currently practice. Qualifications:

Must be organized and detail-oriented. Must be able to communicate effectively both orally and in writing. Must be capable of making independent decisions relevant to the patient's needs. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously. Must function well in atmosphere of stress and interruption.

Requirements:
