ob Overview The Chronic Care Coordinator performs care management for chronically ill patients with any diagnosis of chronic disease. The Chronic Care Coordinator works in collaboration and continuous partnership with chronically ill patients and their family / caregiver (s), clinic providers and community resources in a team approach to increase patients' ability for self-management and shared decision-making. Duties Responsible for registering and validating patients for the CCM program. Conducts 20 minutes of telephonic counseling and education per month to each CCM patient on the roster. Complies with documentation requirements of the Chronic Care Management program by carrying out the care plan with the patient, family/caregiver (s) and providers and recording in the EHR. Monitors adherence to care plans, evaluate effectiveness, monitors patient progress in a timely manner and facilitates changes as needed. Creates an ongoing process for patient and family/caregivers (s) to determine and request the level of care coordination support they desire. Develops and implements innovative marketing and outreach strategies to increase awareness of the Chronic Care Management (CCM) program, drive patient enrollment, and support program growth goals. Identifies new opportunities to expand the CCM program through community engagement, clinic-based outreach, patient education initiatives, and collaboration with providers and staff.
Education:
A minimum of one to three years of experience in a medical clinic or disease management setting preferred, not required.
Knowledge & Skills:
Prior Customer Service experience preferred Working knowledge or willingness to learn medical office management systems and procedures. Has social perceptiveness and service oriented. Competent use of computer systems. Ability to work well in a team environment with also having the ability to work independently. Problem-solving skills to resolve any issues that arise in the clinic. A calm manner and patience working with patients. Maintaining patient confidentiality as per the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Ability to multitask. Superb organizational and planning skills. Ability to work under pressure and react effectively to emergency situations. Passionate about clinical excellence. Willing to help out around the clinic as needed.
