Job Description

The Consortium Student Wellness and Care Coordinator provides non-clinical care coordination, student outreach, resource navigation, wellness education, and referral follow-up for students at Villa Maria College, Niagara University, and Trocaire College. The Coordinator serves as a visible and accessible point of connection, helping students locate and engage with on-campus, community-based, and virtual mental health and wellness resources. Using a coordinated-care approach modeled on the community health worker framework, the Coordinator builds student self-advocacy, promotes wellness and belonging, trains peer ambassadors, and strengthens collaboration across the three institutions. The position is based at Villa Maria College and maintains a rotating in-person and hybrid presence across the participating campuses. This is a non-clinical position and does not provide psychotherapy, diagnose mental health conditions, or replace licensed counseling or emergency services.
ESSENTIAL RESPONSIBILITIES
Provide non-clinical care coordination and individualized resource navigation for referred and self-referred students. Help students identify and access appropriate mental health, wellness, social service, academic support, community, and telehealth resources. Develop and maintain clear referral pathways and follow up with students to support successful connections and continuity of care. Maintain an accessible presence through campus office hours, classroom visits, student events, orientation activities, workshops, tabling, and virtual engagement. Develop low-barrier outreach and programming that normalizes help-seeking, reduces stigma, and strengthens student belonging. Provide workshops and coaching focused on self-advocacy, resource literacy, stress management, coping skills, and wellness. Recruit, train, and coordinate student peer ambassadors or peer wellness coaches in collaboration with student-life personnel. Collaborate with counseling and wellness personnel, student affairs, academic affairs, student life, accessibility services, faculty, staff, and other student-support offices. Partner with faculty and staff to support early identification and appropriate referral of students who may need assistance. Coordinate wellness programming across participating institutions while respecting each campus's identity, systems, and resources. Maintain accurate and confidential documentation of student contacts, referrals, follow-up activities, workshops, events, and outreach. Track engagement, referral completion, program participation, and feedback; prepare periodic program summaries and recommendations. Collaborate with institutional leaders and grant personnel to support external funding and continued development of the consortium model. Follow applicable crisis-response, escalation, privacy, and mandated-reporting protocols.
PREFERRED QUALIFICATIONS
Master's degree in social work, counseling, public health, higher education, student affairs, or a related field. Experience working in higher education or with college-aged, commuter, adult, first-generation, or historically underserved students. Experience with a community health worker, coordinated-care , case management, or student-success coaching model. Experience developing wellness, prevention, self-advocacy, or student-belonging programming. Experience recruiting, training, or coordinating peer educators, mentors, or student ambassadors. Knowledge of mental health, wellness, basic needs, and social-service resources in Western New York. Experience collecting program data, tracking referrals, assessing outcomes, or supporting grant-funded initiatives.
