$27.85 per hour; 35 hours per week schedule

Schedule:

Monday - Friday (some flexibility needed to meet caseload needs)

Position Overview:

The Charles Evans Center is seeking a compassionate and skilled Care Coordinator to join our care coordination team. In this role, you will be a vital member of an interdisciplinary team dedicated to providing care coordination for adults diagnosed with severe mental illness, multiple medical co-morbidities, and/or co-occurring substance abuse disorders.

Key Responsibilities:

Advocate for and support clients in navigating the healthcare system. Engage with community agencies and healthcare providers to ensure clients have access to necessary services. Facilitate wellness self-management and reduce emergency room visits and hospitalizations. Provide expert support and psychosocial/substance abuse interventions. Maintain a focus on client outcomes and best practices.

Qualifications:

Associate's Degree with two (2) years of experience in the human services field; or Bachelor's Degree with one (1) year of experience in the human services field. Clean and valid NYS driver's license. Ability to travel to multiple locations during the day throughout Nassau County.
