NFMMC is looking for an organized Case Coordinator who cares about whole-person health and wants to help patients stay connected, supported, and on track with their medical care. About the Role As a Case Coordinator within the special projects team, you will be the bridge between the Emergency Department and our primary care providers. Your work ensures that every patient leaves the ED not only stabilized but connected to the right follow-up care. You'll be part of a forward-thinking project focused on proactive medical management, improving continuity of care, and reducing avoidable health complications. What You'll Do Schedule primary care and specialty follow-up appointments for patients leaving the ED, targeting appointments within 7 days. Conduct follow-up and reminder calls to increase appointment attendance and ensure continuity of care. Identify barriers to care and help patients overcome them by arranging transportation, referring to community case management programs, and connecting them to needed supports. Collaborate closely with our Population Health team, primary care practices, and other interdisciplinary partners to close care gaps. Track outreach, outcomes, and patient progress in Excel to support ongoing evaluation of the program's impact. Serve as a compassionate, reliable point of contact for patients navigating next steps in their care.

Founded in 1895, Niagara Falls Memorial Medical Center (NFMMC) has a long tradition of serving the Niagara community. NFMMC's hospital and primary stroke program are accredited by the Accreditation Commission for Health Care, and its primary care network is accredited by the National Committee for Quality Assurance. Memorial's inpatient services include a cardiac/stroke unit, a medical-surgical unit, an ICU, a Labor and Delivery unit, and an Adult Inpatient Psychiatry unit, the only one of its kind in Niagara County. Memorial serves as a Center for Community Health by operating three primary care centers, a large outpatient adult mental health clinic, a Health Home, and several innovative community-based services. Thanks to a partnership with Catholic Health, Kaleida Health, and Erie County Medical Center, NFMMC operates the only cardiac catheterization laboratory in Niagara County. Memorial is also home to the Golisano Medical Oncology Center, which operates in collaboration with Roswell Park Comprehensive Care Center.
