Job Description

Office & Patient Care Coordinator Position Overview:

Axzons Homecare is seeking a highly motivated, energetic, and growth-driven Office & Patient Care Coordinator to join our team at the West Hempstead office. In this role, you will act as the face and voice of our agency, serving as the central point of contact for clients, caregivers, and community partners. We are looking for someone with an entrepreneurial mindset—someone who takes full ownership of building the patient census with a proactive, ambitious approach, while delivering exceptionally compassionate and professional service.

Key Qualifications & Language Requirements Bilingual Fluency:

Full professional fluency in both English and Spanish (spoken and written) is required .

Availability & Commitment:

Full-time, on-site role in West Hempstead, NY. Must be vested and flexible to handle urgent client or caregiver calls/issues on weekends when needed.

Business Growth & Census Expansion Proactive Client Acquisition:

Actively drive new patient referrals with an ambitious, "owner-mindset" approach to expanding Axzons' community presence.

Community Engagement:

Build and nurture strong relationships with local healthcare providers, senior centers, family networks, and community leaders (with a dedicated focus on Hispanic community outreach).

Intake & Onboarding:

Efficiently guide new clients and families through the initial intake, assessment, and onboarding processes. 2.

Patient & Caregiver Coordination Liaison & Support:

Serve as the primary contact for patients, families, and caregivers, providing clear, warm, and timely communication.

Issue Resolution:

Actively manage scheduling, service coordination, and emergency inquiries to ensure seamless care delivery.

Weekend Availability:

Remain accessible as needed on weekends to address urgent questions or operational issues from clients or aides. 3. Office Operations & Administration Maintain accurate, up-to-date, and organized client and staff records in compliance with agency standards. Utilize agency software, internal systems, email, and document management systems with high efficiency. Support day-to-day administrative tasks to keep the West Hempstead office running smoothly.

Professional Requirements & Skills Experience:

Prior experience in home care coordination, healthcare administration, customer service, or sales/business development is strongly preferred.

Interpersonal Skills:

Exceptional communication skills, strong emotional intelligence, and a high degree of professionalism.

Organizational Skills:

Excellent multi-tasking capability, strong attention to detail, and goal-oriented time management.

Tech Proficiency:

Comfortable learning and navigating healthcare coordination platforms, office suites, and digital management tools.

Schedule & Location Location:

On-site at the West Hempstead, NY office.

Schedule:

Full-time (Monday-Friday) with required weekend flexibility/availability for urgent client or caregiver needs. Compensation Competitive pay based on experience, with room for growth.

Pay:

$19.00 - $22.00 per hour
