About us Portage Path Behavioral Health is a Behavioral Health Care oupatient and emergency services provider, who providers access to quality Behavioral HealthCare for all. We are a Certified Community Behavioral Health Center in Summit County, Ohio whose mission is to guide people on the path to recovery from mental illness and related substance use issues, empowering them to rebuild their lives and restore their hope.
  • Job Overview:
  • We are seeking a dedicated Client Care Coordinator to join our behavioral health team, and support our Certified Community Behavioral Health Center (CCBHC) Federal SAMHSA grant.
The Client Care Coordinator will play a crucial role in ensuring the smooth coordination of clinical and supportive services for all clients. The Client Care Coordinator will collaborate with clinical and administrative staff at Portage Path Behavioral Health (PPBH) and other community partners and stakeholders within Summit County.
  • Responsibilities:
  • Coordinate the 9 core functions of a Certified Community Behavioral Health Center within PPBH and with community partners and stakeholders.
  • Ability to accurately obtain, track and report data to SAMHSA, clinicians, medical staff, administration and community partners.
  • Ability to complete follow-up communication with community partners at regular established intervals to assess ongoing engagement in outpatient settings.
  • Collaborate with medical and clinical staff to support patient care activities.
  • Works with CCBHC grant program leaders to coordinate patient referrals and treatment.
  • Works with community partners to schedule intake evaluations and referrals for care.
  • Excellent oral and written communication with clients, staff and professionals within the community.
  • Qualifications:
  • Bachelor Degree in healthcare or behavioral health field, required
  • Previous experience in a healthcare setting, preferably in a medical or behavioral health office
  • Strong organizational skills with attention to detail
  • Excellent communication and interpersonal abilities
