As a Senior Placement Specialist at BlueDot Cares, you will play a crucial role in guiding families through the complex process of transitioning their loved ones to appropriate senior living communities. Your responsibilities will include assessing the needs of seniors, providing empathetic support to families, and building strong relationships with healthcare facilities. We are looking for a self-motivated and caring individual with a background in social work or a related field who can make a positive impact on the lives of seniors and their families.Key Responsibilities:
Assessment and Consultation:
Conduct comprehensive assessments of seniors to determine their care needs, preferences, and desires for senior living communities.Client Liaison:
Serve as a liaison between healthcare facilities (skilled nursing facilities and hospitals) and families seeking placement solutions, offering guidance and support throughout the process.Community Research:
Maintain up-to-date knowledge of local independent living, assisted living, and memory care communities, ensuring that you can make informed recommendations to clients.Client Education:
Provide families with information about different senior living options, including costs, services, amenities, and regulations, to help them make informed decisions.Facility Coordination:
Collaborate with healthcare facilities to streamline the transition process, ensuring a smooth and supportive experience for seniors and their families.Empathetic Support:
Offer emotional support and empathy to families during this challenging phase of their lives, demonstrating a compassionate approach.Documentation:
Maintain accurate records of client interactions, assessments, and placement details.Qualifications:
Bachelor's degree in Marketing, Social Work, Psychology, Gerontology, or a related field is preferred but not required. Previous experience in marketing in a senior community, social work, case management, or a related role, with a focus on senior care or healthcare. Strong interpersonal skills and the ability to build trusting relationships with both clients and healthcare professionals. Empathy and compassion for seniors and their families. Self-motivated with the ability to work independently and manage your schedule effectively. Excellent communication skills, both written and verbal. Knowledge of local senior living communities and resources is a plus. Valid driver's license and access to reliable transportation.
