Overview We are seeking a compassionate and proactive Aftercare Coordinator to join our team. In this vital role, you will support survivors transitioning from emergency shelter to stable housing and ongoing stability. Your focus will be on case management, connecting clients with housing assistance programs, and ensuring they receive the necessary social services to thrive. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals from underserved communities, including those experiencing homelessness or mental health challenges. Responsibilities Develop personalized aftercare plans that include housing options, social services, and ongoing support strategies Conduct client assessments using motivational interviewing techniques to identify needs and goals Provide crisis intervention and crisis management support to clients facing high-risk situations Maintain detailed case records using property management tools like OneSite and other case management software Advocate for clients by negotiating with landlords, service providers, and community agencies to secure housing and support services Educate clients on Fair Housing regulations, HUD (Housing and Urban Development) policies, and tenant rights Experience Proven experience in social work, case management, or related social services roles supporting high-risk populations Knowledge of landlord-tenant law, Fair Housing regulations, and HUD guidelines Experience working with individuals experiencing homelessness or mental health conditions in culturally diverse communities Strong skills in negotiation, crisis intervention, and crisis management Familiarity with property management tools such as OneSite or similar platforms Ability to communicate effectively with clients from underserved backgrounds and diverse cultural contexts Experience working within addiction counseling frameworks or supporting clients with substance use challenges Join us in making a lasting impact!
