2997538327-2 Apply for this job now Job Description CVS Health is seeking a Care Management Coordinator for the OhioRISE program to support children and youth with complex behavioral health needs. In this role, you will coordinate care plans, arrange services, and collaborate with families, providers, and community partners to ensure integrated, person-centered support. You will track outcomes, document in care management systems, and help members navigate benefits and resources. Ideal candidates bring healthcare, social work, or behavioral health experience, strong communication skills, and a passion for improving access to quality, equitable care. Responsibilities Coordinate individualized care plans for Ohio RISE members with complex behavioral health needs Collaborate with families, providers, schools, and community agencies to align services Conduct outreach, assessments, and ongoing follow-up to monitor progress and needs Document all member interactions, care plans, and outcomes in care management systems Help members and caregivers navigate Medicaid benefits and community resources Escalate clinical or safety concerns to appropriate clinical staff or supervisors Support transitions of care between inpatient, outpatient, and community settings Use data and member feedback to improve care coordination and service quality Required Skills Care coordination Behavioral health knowledge Case management Healthcare documentation Electronic health record systems Utilization management basics Interdisciplinary collaboration Community resource navigation Regulatory and privacy compliance Data and outcomes tracking
