3035 Kettering Blvd, Moraine, Ohio 45439 Position summary Amani Homecare Services LLC is seeking a compassionate, organized, and dependable Care Coordinator to coordinate quality non-medical homecare services for our clients. The Care Coordinator will serve as an important link between clients, families, caregivers, and the agency office to ensure that clients receive reliable and appropriate care. Qualifications High school diploma or equivalent. Excellent communication and interpersonal skills. Comfortable making phone calls and meeting people in the community. Strong organization and follow-up skills. Ability to work independently. Reliable transportation and willingness to travel locally. Basic computer skills. Healthcare or homecare experience is a plus. Bilingual candidates are encouraged to apply. Valid driver's license and reliable transportation if the position requires community visits.
Responsibilities:
Develop relationships with hospitals, nursing homes, assisted-living facilities, senior centers, physicians' offices, social workers, and community organizations. Identify potential clients who may need homecare services. Follow up with referral sources and prospective clients. Represent Amani Homecare Services at community events. Help increase the number of clients and caregivers. Track marketing activities, referrals, and results. Maintain professional relationships with existing referral partners.
