Value-Based Care Coordinator Job Description:
Support our Axis Healthcare participation in value-based care programs. This role ensures that our patients receive timely, quality care while helping the Axis meet payer program requirements. The candidate must be detail-oriented, proactive, and passionate about improving patient outcomes through coordinated, preventive care.Key Responsibilities:
a. Learn and stay up to date on various value-based care programs, quality measures, and incentive structures from different payers. b. Regularly monitor insurance portals and eCW Healow Insight for patient care gaps, ensuring accurate tracking and documentation. c.Close identified care gaps by:
i. Uploading relevant patient data to payer portals. ii. Scheduling Annual Wellness Visits (AWVs). iii. Conducting patient outreach for Transition of Care Management following hospital discharges. iv. Preparing future visit notes to assist providers in submitting appropriate orders during visits. d. Coordinate between patients, providers, and insurance companies to streamline communication and support compliance with value-based program goals. e. Work closely with clinical staff to ensure necessary screenings, labs, and assessments are completed and documented appropriately.Qualifications:
a. Experience in healthcare administration, care coordination, or medical office support preferred. b. Familiarity with insurance portals and medical documentation is a plus. c. Strong communication skills and ability to engage with patients and clinical staff professionally. d. Proficiency with eCW and general office software. e. Detail-oriented with excellent organizational and multitasking abilities.Pay:
