PeaceHealth Springfield Rehabilitation Hospital seeks a Clinical Liaison to coordinate patient admissions and transitions to our comprehensive rehab services. In this role, you will assess referrals, educate patients and families, and collaborate with physicians, therapists, and community partners to ensure safe, timely transitions of care. You will represent PeaceHealth in the community, support individualized treatment planning, and maintain accurate documentation while advancing our mission of compassionate, holistic rehabilitation in a collaborative, growth-focused environment. Responsibilities Coordinate and evaluate patient referrals for rehabilitation services Conduct patient and family assessments to determine rehab needs and eligibility Collaborate with physicians, therapists, and hospital teams on admission decisions Educate patients, families, and community partners on rehab programs and services Facilitate smooth transitions of care and discharge planning Maintain accurate, timely clinical and referral documentation in EHR systems Serve as a liaison between Peace Health and community providers or facilities Support development and execution of individualized treatment and care plans Participate in interdisciplinary team meetings to review patient progress Promote Peace Health's mission and values through compassionate, holistic care Required Skills Patient assessment Care coordination Discharge planning Clinical documentation (EHR) Interdisciplinary team collaboration Rehabilitation services knowledge Insurance and authorization processes Communication skills Community resource navigation Patient and family education
