Job Summary The Care Coordination Department plays a vital role in ensuring high-quality services, participant satisfaction and support to individuals in various settings. This position requires a compassionate individual who is dedicated to enhancing the quality of life for participants while ensuring their safety and well-being. The team member will work collaboratively with a multidisciplinary team to deliver personalized care plans tailored to each participant' needs. Duties Develop and implement individualized care plans in collaboration with healthcare professionals. Review and confirm all existing and new authorizations. Coordinate participant admissions, including assessments, plan of care creation, and system updates. Monitor and ensure compliance with state, federal, and organizational requirements, including EVV and incident reporting. Manage all critical incidents and ensure compliance with state and federal EIM requirements. Serve as a point of contact for participants, caregivers, and managed care organizations, addressing questions, grievances, and service concerns. Document all pertinent information, correspondence, and service changes. Skills Background in home care services is preferred. Bilingual - English and Spanish, required. Proficient in the use of modern technology, including computers. Excellent communication skills, both verbal and written, for effective interaction with participants, families, and staff. Compassionate approach towards caregiving, demonstrating empathy and patience.
