Local travel required for patient home visits (valid PA driver's license and reliable transportation required) Position Summary Alpha Sante Home Care is seeking a compassionate, detail-oriented Home Care Coordinator / Intake Specialist to serve as the vital link between our patients, their families, our caregivers, and referral sources. This role manages the full intake process for new clients, conducts in-home patient visits, and ensures that all home care aides remain in full compliance with Pennsylvania Department of Health training and documentation requirements. The ideal candidate combines strong clinical or home care operational knowledge with excellent communication, organization, and problem-solving skills. Key ResponsibilitiesIntake & Admissions Serve as the first point of contact for new client inquiries and referrals; respond promptly and professionally to prospective clients, families, hospitals, rehab facilities, and community referral partners Conduct intake interviews to assess client needs, expectations, home environment, and eligibility for services Complete all admission paperwork, service agreements, consents, and required state and payer documentation accurately and on time Verify insurance coverage, Medicaid waiver eligibility (e.g., CHC/OBRA waivers), private pay arrangements, and obtain authorizations as required Develop and document initial plans of care in coordination with clients, families, and clinical staff, ensuring compliance with PA home care regulations (28 Pa. Code Chapter 611) Patient Home Visits Conduct in-home visits with new patients to complete assessments, review the plan of care, and orient clients and families to Alpha Sante's services Perform routine supervisory and quality assurance visits to evaluate patient satisfaction, safety of the home environment, and quality of care being delivered Observe aides in the home to verify services are delivered according to the care plan and agency standards Identify changes in patient condition or needs and communicate updates to the care team, family, and applicable case managers; update care plans accordingly Document all visits thoroughly and in accordance with agency policy and state requirements Caregiver Compliance & Training Oversight Ensure all direct care workers meet Pennsylvania hiring and competency requirements prior to placement, including criminal background checks (Act 34), child abuse clearances where applicable, health screenings/TB testing, and competency examinations Track and maintain current documentation of required aide training, including initial orientation, direct care worker competency, and annual continuing education hours Monitor expiration dates for certifications, clearances, and in-service requirements; notify and schedule aides for renewals before lapses occur Coordinate and, where appropriate, deliver or arrange in-service training and skills refreshers Maintain audit-ready personnel and compliance files for all aides in accordance with PA Department of Health survey standards Address and document performance or compliance concerns, escalating to management as needed Scheduling & Care Coordination Match aides to clients based on care needs, skills, availability, and compatibility; assist with scheduling and coverage for call-offs or emergencies Act as liaison between patients, families, aides, office staff, and external case managers or service coordinators Participate in on-call rotation as assigned to support after-hours coordination needs Administrative & Quality Maintain accurate, confidential client and employee records in compliance with HIPAA Support agency preparation for state licensure surveys, audits, and accreditation reviews Track intake metrics, referral sources, and conversion outcomes; report trends to management Participate in quality assurance and performance improvement (QAPI) activities Perform other duties as assigned to support agency operations and growth QualificationsRequired High school diploma or equivalent; associate's or bachelor's degree in a health or human services field preferred Minimum 1-2 years of experience in home care, home health, or a related healthcare setting (intake, coordination, or supervisory experience strongly preferred) Working knowledge of Pennsylvania home care regulations and direct care worker requirements Ability to pass all required background checks and clearances Valid Pennsylvania driver's license, reliable vehicle, and current auto insurance Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment Excellent verbal and written communication skills; professional and empathetic demeanor Proficiency with scheduling/EMR software and Microsoft Office Preferred LPN, RN, or CNA background Experience with Medicaid CHC waiver programs and managed care organizations (MCOs) Bilingual abilities a plus Physical & Work Requirements Ability to travel locally to patient homes throughout the service area Ability to sit, stand, walk, and climb stairs during home visits Combination of office-based and field-based work Compensation & Benefits Competitive salary based on experience Mileage reimbursement for home visits [Insert benefits: health insurance, PTO, holidays, 401(k), etc.] Alpha Sante Home Care is an Equal Opportunity Employer.
