Part-Time Service Coordinator 30 Hours/Week | Monday-Friday | $21-$22/Hour We're looking for an experienced Service Coordinator who is organized, personable, dependable, and knows how to keep things moving. You'll start with a small caseload and help participants stay connected to the services and supports they need while managing ISPs, HCSIS documentation, provider coordination, monitoring, follow-up, and participant needs. We need someone who can hold the office down. Keep things organized, keep the cases moving, help us grow, and make sure nothing falls through the cracks while leadership is on the move. The Pay Starting Pay:
$21/hour Increase to $22/hour after a successful 90-day review What You'll Do Manage an assigned Service Coordination caseload Develop, review, and update Individual Service Plans (ISPs) Complete participant documentation in HCSIS Coordinate services with participants, families, and providers Monitor services, participant needs, goals, health, safety, and satisfaction Follow up on authorizations, service issues, and plan changes Conduct required participant contacts and face-to-face visits Help resolve barriers and keep services moving Maintain organized and current participant records You Must Have Previous Pennsylvania Service Coordination experience Working knowledge of HCSIS Experience developing, reviewing, and managing ISPs Knowledge of waiver services, service authorizations, participant monitoring, and provider coordination Strong documentation and follow-up skills A valid driver's license Reliable personal transportation A satisfactory driving record Ability to travel to participant homes and meetings as needed Education/Experience Requirement You must also meet one of the following: A Bachelor's degree that includes at least 12 college-level credits in sociology, social welfare, psychology, gerontology, or another behavioral science; OR A combination of education, training, and experience totaling four years, including at least 12 semester hours of relevant social-science coursework Who Fits Our Culture?
