Care Coordinator Open Arms Home Care | Philadelphia Area Open Arms Home Care is actively seeking a Care Coordinator to join our home care team. The ideal candidate is experienced, organized, and dependable, and will be responsible for coordinating services between clients and caregivers. We offer a competitive salary of $45,000-$55,000 (commensurate with experience), vacation, and a safe working environment.

RESPONSIBILITIES

Support care planning for clients with complex and chronic conditions, including performing and arranging for waiver services. Refer clients to community resources, agencies, and organizations to enhance and supplement services for the client. Document progress notes related to client contacts, advocacy, and care in accordance with Open Arms Home Care policies. Work with appropriate resources on evaluations for clients in need of discharge planning, transitions of care, or additional services. Facilitate authorizations and orders to coordinate services for clients. Develop and maintain relationships and knowledge of applicable community services. Facilitate patient access to recommended services.

QUALIFICATIONS

Minimum 2 years' experience in Home Care or a related field. Knowledge of Home Care Regulations and timelines. Excellent time management skills and ability to work as part of a team. Working knowledge of office equipment, such as computers, scanners, copiers, printers, and fax machines. Open Arms Home Care is an Equal Opportunity/Affirmative Action employer.
