Position Summary:
The Referral Coordinator is responsible for coordinating specialty care for patients in need of external consultation as requested by the primary care physician. The Referral Coordinator will represent the company professionally and positively to enhance and promote the core values and mission of PHA-Adult Medicine, always exercising utmost discretion, diplomacy and tact in customer interactions. The Referral Coordinator works under the primary supervision of the Medical Director.
Duties and Responsibilities:
Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety. This tracking may use an IT database. Ensure complete and accurate registration, including patient demographic and current insurance information. Assemble information concerning patient's clinical background and referral need Per referral guidelines, provide appropriate clinical information to specialists. Contact review organizations and insurance companies to ensure prior approval requirements are met. Present necessary medical information such as history, diagnosis and prognosis. Review details and expectations about the referral with patients. Assist patients in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance). Serve as the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns. May assume advocate role on the patient's behalf with the carrier to ensure approval of the necessary supplies/services for the patient in a timely fashion. Identify and utilize preferred specialists for insurance companies when making referrals. Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers. Ensure that referrals are addressed in a timely manner. Remind patients of scheduled appointments via mail or phone, EMR messaging. Work with primary care team to ensure that patient's primary care chart is up to date with information on specialist consults, hospitalizations, ER visits and community organization related to their health. Process request for medical records. Assist with Post Hospital/SNF Transition of Care efforts including reaching out to patients, scheduling appointments to be seen by primary care office. Assist when necessary with other administrative duties such as check in or check-out. Respect patients' right to privacy and confidentiality Understand and follow HIPAA guidelines Perform other duties as assigned. Knowledge, skills and abilities: Ability to be punctual and dependable Ability to be nice and friendly to our patients Ability to prioritize and perform duties efficiently; adjust to and accept the unexpected Effective verbal/written communication skills; excellent telephone manners and etiquette Knowledge of medical terminology Knowledge of various insurance plans eg. Medicaid, Medicare and private insurances. Qualifications:
High school diploma or GED equivalent required Medical Assistant Certification preferred with two years experience in a primary care practice setting Experience with EHR system preferred Insurance verification experience preferred
Pay:
$20.00 - $21.00 per hour Expected hours: 36.0 per week
