This is accomplished by increasing residents' access to services and community resources and by facilitating their participation in programs that enhance their physical, social, and mental well-being. The ideal candidate will possess a blend of administrative expertise and hands-on experience in social work with the older adult population and/or LIHTC senior housing buildings. Responsibilities Acts as liaison between residents and building management and manages the balance between advocating for the resident and supporting management. Supports management to ensure resident compliance with facility regulations and policies. Provides comprehensive assessments to all residents needing assistance. Develops, implements, and monitors resident care plan which reflects information received during the assessment process. Maintains case recordings, clearly, concisely and in a timely manner. Provides monthly reports on Service Coordination activity to Management and CIP as needed. Provides and distributes monthly resident newsletter. Provides assistance (counseling, etc.) to residents with problem resolution that comes up as part of the care management process. Maintains up-to-date information on community resources. Refers/arranges for and monitors community services including benefits and entitlements for residents. Coordinates with Center in the Park to provide or arrange programs for residents, as well as develops and coordinators appropriate resident groups. Monitors continuity and continued appropriateness of services for residents whether initiated or pending on a waiting list. Sets-up volunteer support programs with service organizations in the community to benefit residents. Experience proficient in use of Microsoft applications ability to work well independently and as part of a team Demonstrated expertise in program development and coordination Strong relationship management skills to effectively engage clients, families, and community partners. Background in administrative functions related to social work practices is highly desirable.
