Monday through Friday, 8:00 AM - 5:00 PM Position Summary We are seeking a highly motivated and experienced Bilingual Care Coordinator (English/Spanish) to join our Home Health team. The ideal candidate will have a strong background in home healthcare coordination, excellent communication skills, and experience working with caregivers, patients, and clinical staff to ensure high-quality care and service delivery. Responsibilities Coordinate and manage patient care services in accordance with physician orders and agency policies. Serve as a liaison between patients, families, caregivers, clinicians, and referral sources. Schedule and coordinate Home Health Aides (HHAs) and other care staff. Maintain accurate patient records and documentation. Monitor patient service plans and ensure continuity of care. Assist with intake, staffing, and ongoing patient care coordination. Communicate effectively in both English and Spanish with patients, families, and staff. Utilize agency software systems to manage scheduling, documentation, and reporting. Ensure compliance with all applicable regulations and company policies.
QualificationsRequired:
High School Diploma or GED. Fluent in English and Spanish (written and verbal). Minimum of 2 years of experience in a Home Health agency. Experience coordinating and scheduling Home Health Aides (HHAs). Experience using the WellSky Home Health software system. Strong organizational, multitasking, and communication skills. Proficiency with Microsoft Office and general computer applications.
Preferred:
Some college coursework or an Associate's degree in Healthcare Administration, Business Administration, Nursing, or a related field. Additional experience in patient care coordination, healthcare administration, or case management. How to Apply Interested candidates should submit their resume and qualifications for consideration.
