Job Description

Position Summary:
Responsible for care coordination activities in assigned program components. Develops plans, tools and implements processes for coordination of care, analysis, and improvement of clinical and service outcomes for Southeast Florida Behavioral Health Network (DCF Managing Entity).
Required Education/Experience:
  • Four-year college degree or university program certificate
  • Minimum of three years in behavioral health
  • Understanding of basic principles of care coordination
Essential Job Functions:
Oversees team‑based and specialty programs, ensuring effective implementation by monitoring performance indicators, reviewing program data and deliverables, and maintaining fidelity to evidence-based models while promoting high quality, client centered services. Monitors waitlists, service needs, gaps and redirects resources as needed Assists NSP participants in navigating the full continuum of care by coordinating seamless transitions between varying levels of care, ensuring services remain responsive to everyone's evolving needs. Represents SEFBHN, in its capacity as the Managing Entity, at community and stakeholder meetings to ensure consistent communication, strengthen partnerships, and support coordinated responses across the behavioral health system. Collaborates with NSPs by organizing and participating in multidisciplinary team meetings and case staffings for individuals designated as high utilizers or belonging to priority populations, ensuring comprehensive care planning across systems of behavioral health. Develop partnerships and agreements with community partners to leverage resources and share data. Works with direct care staff to create and coordinate diversion strategies, including proactive crisis planning, to help individuals remain safe in the community and avoid unnecessary higher‑level placements by connecting them to appropriate behavioral health services. Track the progress of individuals served using the Coordination of Care (CoC) Module. Coach and promote High Fidelity Wraparound and Suicide Prevention (QPR/QPRT) by developing proficiencies in both. Provide technical assistance to NSPs and assist in eliminating system barriers. Collaborates with both children's and adult service systems to promote best practices in family engagement and strength‑based recovery, serving as a liaison to entities such as school districts, child welfare, adult protective services, the justice system, and the Agency for Persons with Disabilities. Ensure NSPs utilize quality assurance processes to review program standards and utilization. Ensure medical necessity of treatment recommendations is met by requiring the use of Level of Care and Functional assessments. Maintain working knowledge regarding the system of care continuum services. Serve as a subject matter expert at multidisciplinary meetings (i.e., Review Teams, Safety Support Teams, Keeping Families Connected and/or Lockout staffings). Reviews, tracks, and provides a first level approval of Transitional Vouchers throughout the NSP as needed on a timely basis. Assists the PPOCs by contributing SME‑level guidance during Service Delivery Narrative reviews, BASE system updates, and validation process to ensure accuracy and compliance. Uses data to develop, analyze, and interpret dashboards to improve program oversight, inform decision‑making, and track key performance outcomes. Completes other job‑related tasks as assigned. Reinforces MOUs and Agreements for partnerships within the assigned programs. Prepares reports as required. Community resource awareness. Working knowledge regarding substance use and mental health recovery-oriented systems of care continuum. Knowledgeable about system mandates, relevant legislation, best practices, and DCF Guidance documents. Knowledge of available funding streams. Strong interpersonal skills. General Problem Solving
