About the Position Harmonie House is seeking an experienced Care Coordinator / Case Manager to independently manage a caseload of clients receiving home-based and community services. Assessment and person-centered care planning are a primary focus of this position. You will conduct in-person assessments, identify each client's functional needs, risks, strengths, preferences, and goals, and develop comprehensive individualized care plans that clearly guide their services and supports. This is a caseload ownership position. You will be fully responsible for the clients assigned to you, including assessments and care planning, intake and enrollment, quarterly monitoring, scheduling, chart audits, care documentation, data entry, incident reporting, service coordination, and ongoing communication with clients, families, caregivers, and other members of the care team. We are looking for someone who is independent, proactive, organized, and confident taking ownership of their work. You should be able to manage your caseload without constant direction, recognize what needs attention, anticipate deadlines, solve problems, and follow issues through to resolution. What You'll Do Conduct initial and ongoing in-person client assessments and reassessments . Assess ADLs/IADLs, functional abilities, assistance and supervision needs, health and safety risks, support needs, strengths, preferences, and goals. Develop and maintain comprehensive, individualized person-centered care plans , including meaningful goals, interventions, and support strategies. Ensure care plans provide clear direction for caregivers and accurately reflect each client's needs, choices, preferences, and authorized services. Manage new-client intake and enrollment , including accurate setup and data entry in the EHR. Maintain complete and accurate client records, care documentation, authorizations, and required information. Conduct and document required quarterly monitoring and follow up on identified needs or concerns. Manage client and caregiver schedules , service changes, and coverage needs. Complete routine chart audits and follow up on missing, incomplete, or inconsistent documentation. Complete and follow up on incident reports , client concerns, and changes in condition or circumstances. Monitor service delivery and authorizations and identify discrepancies or needed changes. Coordinate with clients, families, caregivers, Case Management Agencies, nurses, and other members of the care team. Maintain responsibility for required deadlines and ensure assessments, care plans, monitoring, documentation, and follow-up remain current for your caseload. Identify problems, take appropriate action, communicate concerns, and follow issues through to resolution . Participate in quality assurance and regulatory activities as needed. What We're Looking For We are specifically seeking someone with hands-on experience conducting client assessments and developing individualized plans of care , rather than someone whose experience is primarily administrative case management. Experience in home health, IHSS, Medicaid-funded services, IDD services, long-term care, PASA, HCBS, or a similar care environment is highly valued. Strong candidates will have experience with functional-needs and
ADL/IADL
assessments, person-centered care planning, developing goals and interventions, EHR documentation, client monitoring, service coordination, and independently managing an ongoing caseload. Most importantly, we are looking for someone who takes ownership of their clients and their work. When a client is assigned to you, you know what is happening with that client, what is due, what needs attention, and what needs to happen next. Qualifications 2+ years of relevant care coordination, case management, home health, IHSS, Medicaid, disability services, or similar experience preferred. Demonstrated experience conducting client assessments and developing individualized care plans strongly preferred. Experience independently managing an ongoing client caseload strongly preferred. Bachelor's degree in social work, human services, healthcare, psychology, nursing, or a related field preferred; directly relevant experience may substitute for education. Strong assessment, interviewing, care-planning, documentation, and organizational skills. Ability to work independently, manage competing priorities, and consistently meet deadlines. Strong problem-solving skills and attention to detail. Strong written and verbal communication skills. Comfortable learning and working within EHR and other technology systems. Valid driver's license, reliable transportation, and ability to travel to client homes throughout the assigned service area. Location & Travel This is a remote/home-based position for an employee located in Grand Junction or the surrounding area. Administrative work is completed remotely. This is not a remote-only position. Regular in-person client contact is an essential part of the role. Travel is required for assessments, reassessments, quarterly monitoring, and other client needs. Compensation $28.00-$34.00 per hour, based on directly relevant experience. Compensation within the range will reflect assessment and care-planning experience, relevant home health/IHSS/Medicaid experience, and demonstrated ability to independently manage a client caseload. Pay:
$27.00 - $34.00 per hour Expected hours: 32.0 - 40.0 per week
