Sentara is hiring a Care Coordinator for ourPopulation Health Ambulatory Clinical Support Team Location:
Sentara Quality Care Network Charlottesville Status:
Full Time (40 hours/week)Shift:
Days (Anticipated 8AM-5PM)- This Hybrid position requires 2-3 days per week in the office, in addition to days on site at affiliate practices, and allows for some work from home days.
- Ideally seeking a licensed Pharmacy Technician with a Certified Medical Assistant (CCMA) or LPN
- Virginia Pharmacy Tech License and CPhT License (Received through the NHA or PTCB) OR Certified Medical Assistant (CMA), or Registered Medical Assistant (RMA), or Certified Clinical Medical Assistant (CCMA) OR LPN Virginia state license Experience 1-year of Long-Term Care experience (Required) Ambulatory and Clinical Pharmacy experience (Preferred) 1-Year Medicaid or Health Plan experience (Preferred)
