Job Description
Job Overview Point Healthcare Services LLC is seeking compassionate, driven individuals to join our team of Case Managers. We work directly with Medicaid and Medicare programs such as HealthCare Authority (HCA), and Washington State Department of Health Services(DSHS) through the Health Home Program. As a Case Manager, you will work directly with clients in their homes and communities—building strong relationships and helping clients connect to care, overcome real-world barriers, and build more stable, supported lives. You will support individuals navigating chronic health conditions, behavioral health challenges, and social barriers such as housing and access to services. Your role is to provide consistent support, coordinate care, and help clients navigate systems that can be difficult to manage on their own. This is a non-clinical, field-based role focused on coordination, advocacy, and relationship-building. Duties Build trust-based relationship with clients through monthly in home visits. Conduct thorough client assessments to identify needs across medical, social, and behavioral health domains. Perform outreach calls to schedule home, facility, and hospital visits. Care planning including Health Assessments Plans (HAPs) and Health Risk Assessment (HRA). Develop individualized client centered care plans that incorporate medical treatment, social services, and community resources. Support clients navigate benefits, treatment plans, and community resources to bridge the gap in healthcare. Coordinate with hospital staff, social workers, physicians, and external providers to ensure continuity of care. Facilitate discharge planning by arranging post-acute services, home health care, or follow-up appointments as needed. Maintain accurate documentation within EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems while adhering to HIPAA regulations to track clients progress and outcomes. Communicate effectively with clients and their families, providing education on treatment plans and available resources. Participate in multidisciplinary team meetings to review patient progress and adjust care strategies accordingly. Skills Knowledge of managed care processes, health insurance policies, and utilization review procedures. Experience working with diverse populations from various cultural backgrounds; bilingual skills are a plus. Familiarity with EMR/EHR systems such as Epic or Cerner; proficiency in Microsoft Office applications for reporting and communication. Strong understanding of HIPAA compliance and patient confidentiality standards. Ability to perform discharge planning effectively while coordinating healthcare, social services and behavioral health resources. Excellent client communication skills with a focus on empathetic listening and clear information sharing. Minimum Qualifications (Required) ✔ Bachelor's degree in: Social Work Psychology Human Services Behavioral Sciences Or a closely related field OR ✔ Certified Medical Assistant (CMA or RMA) Associate's degree required Preferred Experience Case management or care coordination Housing or social services Behavioral health or substance use support Experience working with Medicaid or Medicare populations Bilingual (Spanish, Somali, Vietnamese, Amharic, etc.) Requirements Washington State residency Valid driver's license and reliable vehicle Ability to travel for field visits Comfortable using EMR systems and technology Why Join Point Healthcare Services?
