Chronic Care Management (CCM)

Coordinator Location:

Remote (Work From Home) with a mandatory in-office meeting once every two weeks at our Farmington Hills, Michigan clinic.

Compensation:

$15.00 - $18.00 per completed CCM call/interaction, depending on experience.

Position Overview:

We are seeking a seasoned, organized, and deeply empathetic Chronic Care Management (CCM) Coordinator to join our Primary Care and Sports Medicine practice. In this role, you will work entirely from home to manage monthly care coordination and phone consultations exclusively for our geriatric and qualifying chronic care Medicare patient populations.

Please note:

This CCM role focuses entirely on chronic care and senior patients; it does not handle auto accident or personal injury accounts. --- Key Responsibilities

Conduct Monthly Care Calls:

Call enrolled Medicare/geriatric patients monthly from your home office to review care plans, track health goals, and address chronic conditions.

Document Care Minutes:

Accurately log clinical coordination time and updates within our NextGen EHR system to ensure strict Medicare billing compliance.

Identify Patient Needs:

Act as a critical clinical bridge between remote patients at home and our clinic team, flagging medication issues, changes in health status, or home care gaps.

Bi-Weekly Office Meetings:

Travel to our Farmington Hills office once every two weeks for mandatory team alignment, training updates, and provider feedback. --- Requirements & Qualifications

Experience:

Minimum of 5 years of experience in healthcare, chronic care management, care coordination, or geriatric patient care.

EHR Software:

Experience with Epic or Practice Fusion is highly preferred (training will be provided for our native NextGen system).

Target Knowledge:

Deep understanding of Medicare guidelines, preventative wellness benchmarks, and the unique physical and communication needs of a senior population.

Pay:
