Job Description
Clinical Care CoordinatorPosition Summary The Care Coordinator works directly with individuals experiencing behavioral health, substance use, housing, medical, and other social-service needs. This position is responsible for assessing client needs, developing individualized care plans, coordinating services, and helping clients overcome barriers to long-term stability, independence, and recovery. The ideal candidate is compassionate, organized, resourceful, and comfortable collaborating with treatment providers, community organizations, government agencies, housing programs, peer specialists, and other members of a client's support system. Essential Duties and Responsibilities Conduct client outreach, screenings, ASAM assessments, and ongoing follow-up. Develop and update individualized care plans based on each client's identified needs, strengths, and goals. Connect clients with behavioral health treatment, substance use services, medical care, housing resources, benefits, transportation, employment assistance, and other community supports. Coordinate referrals and warm handoffs to service providers. Advocate for clients and assist them with navigating complex healthcare and social-service systems. Collaborate with treatment providers, hospitals, peer specialists, recovery residences, community agencies, and family or support persons when appropriate. Provide supportive counseling, motivational support, crisis intervention, and problem-solving assistance. Monitor client progress and follow up on referrals, appointments, applications, and care-plan objectives. Assist with discharge planning and transitions between levels of care or service providers. Maintain accurate, timely, and compliant documentation in electronic health record and care-coordination systems. Participate in case consultations, team meetings, training, quality-assurance activities, and program reporting. Maintain professional boundaries and comply with HIPAA, confidentiality, ethical, and organizational requirements. Minimum Qualifications Master's degree in social work, mental health counseling, marriage and family therapy, or a related behavioral health field. Must hold one of the following active Florida credentials: Registered Clinical Social Work Intern (RCSWI) Registered Mental Health Counselor Intern (RMHCI) Registered Marriage and Family Therapist Intern (RMFTI) Licensed Clinical Social Worker (LCSW) Licensed Mental Health Counselor (LMHC) Licensed Marriage and Family Therapist (LMFT) Previous experience in care coordination, case management, behavioral health, substance use treatment, social services, housing services, or a related setting is strongly preferred. Knowledge of community resources and public-benefit systems is preferred. Strong communication, documentation, organization, and time-management skills. Ability to manage multiple clients and competing priorities in a fast-paced environment. Ability to work independently while collaborating effectively with an interdisciplinary team. Comfortable working with individuals experiencing complex behavioral health, medical, housing, legal, and socioeconomic challenges. Ability to maintain a private, professional home workspace and reliable internet connection. Collaborative, flexible, and responsive approach to teamwork and client care. Schedule and Work Environment Full-time position, Monday through Friday, 9:00 a.m. to 5:00 p.m. Fully remote, work-from-home position. Employees are expected to remain accessible and responsive to clients, colleagues, providers, and community partners during scheduled working hours. The Care Coordinator will join a dynamic, collaborative, and responsive team committed to supporting one another and providing high-quality client care. Compensation Registered Interns RCSWIs, RMHCIs, and RMFTIs begin at an annual salary of $70,000 . Following successful completion of the 90-day introductory period, registered interns may be eligible for a salary increase to $75,000 , based on performance and fulfillment of position expectations. Fully Licensed Professionals LCSWs, LMHCs, and LMFTs begin at an annual salary of $85,000 . Following successful completion of the 90-day introductory period, fully licensed professionals may be eligible for a salary increase to $90,000 , based on performance and fulfillment of position expectations.
