Job Summary:
The Health Care Coordinator (HCC) will utilize a family-centered and strength-based team approach, with community involvement, in the education and service provision of children and families who are involved with the Family Preservation and all in-home preservation programs. The HCC will be responsible for assessing, coordinating, monitoring, educating, documenting, and tracking client health care needs of children and their caregivers. The Health Care Coordinator will serves as a consultant to Family Preservation staff and provides direct service to families in the community. This position also reviews referrals by medical providers for accurate coordination of intervention services. This position will refer cases to the proper community resource providers and/or committees for mental health funds/services based on assessment and screening tool results. The HCC is responsible for ensuring cross-system integration and interdisciplinary practice to support social and emotional development, substance abuse recovery, wellness, and child development. This position may be responsible for outreach activities to local hospitals or similar programs.Essential Job Duties:
Maintain supervision of assigned clients; including assessing, educating, coaching, referring, and connecting. Maintain communication with family health care providers and referral stakeholders from differing organizations to meet the family and child needs according to the developed health plan. Maintain client, meetings, documentation, and other data. Coordinate and conduct special case staffing. Outreach with local hospitals Attend program services meetings and all family preservation training. Perform other duties as neededMinimum Qualifications:
Education:
Certified Nursing Assistant (CNA), Licensed Practical Nurse (LPN), Registered Nurse (RN)- preferred.Experience:
Proven combination of education and experience which demonstrates the ability to lead, manage, enhance, and promote the assigned programs of the agency. Experience in home visiting is preferred. Licensure, Certification, and/orRegistration:

Review cases and assess wellness and healthcare needs Establish and maintain effective working relationships with families, DCF employees, Preservation employees, local hospitals, and other community agencies, as needed. Maintain accurate and timely records to ensure accountability of assessments and data. Enter notes in FSFN within 48 hours of each contact. Work efficiently using time management skills to make contact with families and obtain assessments within specific timeframes Communicate effectively with management, staff and clients. Be organized and detail-oriented.Knowledge of:
Florida Statutes pertaining to Child Welfare Family Dynamics Family Preservation Protocols, Social Worker principles, techniques, and practices and their application to complex case work, group work, and community problems.Other Requirements:
