We are seeking an InformationReferral Specialist to provide information to our consumers in response to service and eligibility inquiries. What you'll do: Assess the health and functional status of members Develop community- based care plans and related service packages Participate in the development of a member's Individual Care Plan (ICP) Arrange, coordinate, and monitor the provision of appropriate members participate in the determination of a member's need for institutional and community long term care services What you'll bring: Your passion for helping others and desire to make a difference. Bachelor's degree in social work or related field highly preferred; current licensure if appropriate Minimum of two years' professional experience in the care of people 65 and older Current Social Work license (LSW) or higher preferred Working knowledge of social, emotional, and physical process of aging Ability to obtain information of a personal nature through interviews Ability to gain and hold the confidence of clients and their families Draw appropriate conclusions and make recommendations Knowledge and skill necessary to work successfully within an interdisciplinary team Excellent written and verbal skills.
