- 60169 & 60192 Job Overview We are seeking a dedicated LTSS (Long-Term Services and Supports) Service Care Coordinator to join our healthcare team.
This vital role involves coordinating comprehensive care plans for individuals requiring long-term assistance, ensuring seamless integration of services across medical, social, and community resources. The ideal candidate will possess a strong background in healthcare coordination, with expertise in managing diverse patient needs within complex care environments. This position offers an opportunity to make a meaningful impact on patient outcomes by facilitating effective communication, discharge planning, and resource management. Responsibilities Conduct intake assessments and facilitate discharge planning to ensure smooth transitions between care settings. Utilize EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems to document patient information accurately and efficiently. Manage utilization review and utilization management processes to optimize resource use while maintaining compliance with healthcare regulations. Work with individuals from diverse cultural backgrounds, demonstrating sensitivity and effective communication skills. Support case management activities by assessing patient needs, coordinating services, and monitoring progress. Ensure compliance with HIPAA regulations and maintain confidentiality of patient information. Conduct patient visits to complete risk screenings and assessments Requirements A Bachelor's degree or Human Services or a related field At least an Active and unrestricted LPN license (RN License is preferred) A certified or licensed Social Worker is acceptable 1 year of experience conducting comprehensive assessments and providing formal services for the elderly 1 year of experience working with patients with behavioral and/or mental health issues Proficiency in utilizing
Microsoft Office:
