Job Description
Overview B&D Integrated Health Services is seeking a full-time Tailored Care Manager Extender. Under the direction of the Tailored Care Management Supervisor, the Tailored Care Manager Extender (CME) is responsible for providing medical care management services to a specified panel of patients within the clinical setting. The primary responsibilities of the CME are to promote the whole person, and integrated care by planning, coordinating, tracking, closing gaps in care, and monitoring care to individuals. The CME works in coordination with CM to facilitate the integration of physical health, behavioral health, and community supports through an intentional and ongoing engagement of multidisciplinary providers. TCM Services can take place face-to-face, telehealth, or via phone. What is Tailored Care Management? TCM is a service designed to connect clients with local providers/resources which will address their needs as it relates to their physical and mental health and environment while promoting self-sufficiency and independence. What is the goal of Tailored Care Management? To assist clients with coordinating care within ALL aspects of life to improve their individual functionality and reduce dependence on physical and mental providers. What does a Tailored Care Manager Extender do? Extenders may support Care Managers in delivering TCM by performing activities that fall within the below categories: Performing general outreach, engagement, and follow-up with members. Coordinating services/appointments (ex. Appointment reminders, arranging transport) Engaging in health promotion activities and knowledge sharing. Sharing information with the CM and other members of the care team on the client's circumstances, including gathering information about the client's progress towards their goals. Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services. Participating in case conferences. Support the CM in assessing and addressing unmet health-related resource needs.
