Urgent Care Practitioner Location :
Clinical Assessment Service Band 6 Part time considered You should be a registered Health Care Professional, and have experience in the delivery of health and/or social care, ideally in an urgent or emergency care setting. We are looking to recruit dynamic individuals that will compliment the existing team of multi-professional practitioners. Main duties of the job The Urgent Care Practitioner (UCP) in the Telephone Clinical Assessment Service is responsible for providing safe, timely, and clinically appropriate assessment and triage of patients contacting the service.Key responsibilities include:
Conducting structured telephone assessments to determine the urgency of patient needs. Advising patients on the most appropriate care pathway, including self-care, referral to primary care, or escalation to urgent or emergency services. Documenting consultations accurately in the patient record system to ensure continuity of care. Identifying and responding to safeguarding concerns and clinical red flags. Supporting the service in meeting clinical quality, governance, and patient safety standards. Communicating effectively with multi-disciplinary teams to coordinate patient care. This role is critical to maintaining safe access to urgent care, reducing ED attendances, and ensuring patients receive timely clinical advice and intervention On occasion you may be required to support other urgent care services such as UTC's & home visiting which run 24/7 services. There is daily GP support available remotely. Make clinical decisions within agreed protocols, providing appropriate advice or escalation. Document all consultations clearly and accurately in the patient record system. Identify and act upon safeguarding concerns or clinical red flags. Communicate effectively with patients, carers, and multi-disciplinary teams to ensure continuity of care. Participate in clinical governance, quality assurance, and ongoing professional development activities The post holder will be expected to model the professional attitudes and behaviours outlined in the LCHS behaviours framework. Please see job description for further details. The service operates 24 hours a day, 365 days a year and the post holder will be expected to work flexibly. Person specification Qualifications Essential criteria Registered health care professional with existing registration Evidence of degree level study or equivalent Level 6/7 History Taking qualification or a willingness to work towards. HEI or LCHS approved minor injury or a willingness to work towards HEI or LCHS approved minor illness course, or a willingness to work towards Desirable criteria Supervision/mentoring training or qualification Leadership/management training or qualification Independent prescribing / supplementary prescribing qualification. HEI or LCHS approved paediatric course Experience Essential criteria Substantial consolidated experience of clinical practice in a range of setting including assessing minor illnesses/injuries face to face Administration of medication within Patient Group Directives (PGD) Experience of working in a multi-professional/multi agency environment Experience of coaching and mentoring others Desirable criteria Experience of urgent or emergency care Ability to demonstrate a working knowledge of the local clinical governance agenda Previous experience of supporting or managing change Previous experience working remote consultation Enjoy a better quality of life in one of the UK's most welcoming counties.
