Northeast Florida is home to more than 425,000 active-duty servicemembers, veterans, and their families. While numerous resources exist to support the veteran community, navigating federal, state, local, and nonprofit systems can be complex and overwhelming. Mission United exists to bridge that gap by connecting veterans and their families to the services, benefits, and community supports they need to achieve long-term stability and successfully transition to civilian life. The Veteran Care Coordinator (VCC) serves as a trusted navigator and advocate within the Mission United program, providing individualized care coordination to veterans, active-duty servicemembers, and their families throughout Northeast Florida. This position conducts outreach, completes comprehensive intake assessments, determines program eligibility, identifies barriers to stability, and develops coordinated plans to connect participants with appropriate VA benefits, healthcare, housing, employment, financial stability resources, behavioral health services, and other community supports. The Veteran Care Coordinator works collaboratively with internal United Way programs, community partners, to ensure veterans receive seamless, compassionate, and timely assistance. Through ongoing follow-up, documentation, advocacy, and relationship building, this role helps improve outcomes for veterans while advancing Mission United's commitment to strengthening the regional network of care. Essential Functions & Responsibilities Veteran Care Coordination Conduct outreach, assess participant needs and eligibility, and provide individualized care coordination to veterans, servicemembers, and their families. Maintain an active caseload by developing service plans, coordinating referrals and warm handoffs, conducting follow-up, monitoring progress, and documenting outcomes. Connect participants with VA healthcare and benefits, housing, employment, financial stability, behavioral health, and other community resources. Provide compassionate, veteran-centered support while exercising sound judgment, professionalism, and cultural competence. Collaboration & Community Engagement Collaborate with internal programs, the VA, community partners, and other stakeholders to coordinate services and strengthen the regional network of veteran support. Represent Mission United through outreach events, resource fairs, community partnerships, and other veteran engagement activities. Program Operations & Professional Excellence Maintain accurate, timely, and audit-ready participant and program documentation to support reporting, grant compliance, performance measurement, and continuous improvement. Meet assigned deadlines and program deliverables while maintaining organized records, referral systems, and resource information. Maintain knowledge of veteran resources, military culture, community services, organizational policies, and grant requirements through ongoing training and professional development. Contribute to a collaborative, accountable, and service-focused team environment and perform other duties as assigned.
Experience/Qualifications :

Education & Experience United States Veteran with any discharge status other than dishonorable. High school diploma or equivalent required. Bachelor's degree in Social Work, Human Services, Psychology, Public Administration, Counseling, or a related field preferred. Equivalent combinations of education, military service, certifications, and directly related professional experience will be considered. Experience providing care coordination, case management, community navigation,, or human services preferred. Experience working with veterans, military service members, military families, or military-serving organizations preferred. Knowledge Working knowledge of Veterans Administration (VA) healthcare, disability compensation, pension, education, employment, and other veteran benefit programs preferred. Knowledge of community resources and social service systems that support veterans and military families. Professional Competencies Ability to establish rapport, build trust, and foster collaborative relationships with veterans, military families, community partners, and multidisciplinary teams. Strong interpersonal, verbal, written, and customer service skills. Strong critical thinking, problem-solving, organizational, and time management skills with the ability to effectively manage competing priorities. Ability to engage individuals experiencing transition, or complex life challenges with empathy, professionalism, and sound judgment. Ability to maintain accurate documentation and effectively utilize electronic case management systems, Microsoft Office applications, and other technology platforms. Ability to work independently while contributing to a collaborative, team-oriented environment. Demonstrated professionalism, sound judgment, adaptability, and commitment to maintaining confidentiality. Additional Requirements Ability to pass and maintain a Level II background screening. Successful completion of a pre-employment drug screening. Valid driver's license, reliable transportation, and current automobile insurance if driving on behalf of United Way of Northeast Florida. Ability to maintain reliable high-speed internet service sufficient to perform job responsibilities in a hybrid or remote work environment. Flexibility to work variable shifts including occasional evenings or weekends to support community outreach events, trainings, or organizational initiatives, as needed.
