Are you looking for an excellent opportunity to work with a faith-based non-profit organization? BeWell@Home is currently recruiting for a full-time Wellness Coordinator. We are seeking someone who is a compassionate, dedicated, and caring professional to join our team! The Wellness Coordinator will plan, organize, develop and conduct all assessments of the health and overall needs of the BeWell@Home members, their non-qualifying spouses, and prospective members. Develops individual wellness plans, individual care coordination plans for care services, conduct wellness interventions, and oversee the implementation of all the plans. Establishes professional relationships with providers and oversee the services delivered to members. Provides education and assistance to members as needed. Assists with all other program operations including administrative needs.
QUALIFICATIONS
Bachelor's degree in Social work, Psychology, Gerontology, Human Services, Nursing or Occupational Therapy. Master's degree preferred or equivalent experience in continuing care at home program or senior living.
REQUIREMENTS
Knowledge of the principles, methods, and standards of professional care coordination Familiar with organization, regulations, policies and procedures Must be able to take evening and weekend call on a rotating basis Must be able to interact with management, staff, members, residents, and the general public in a professional manner Must be able to utilize technology appropriately as needed to meet the requirements of the position Must possess leadership ability and willingness to work harmoniously with campus personnel Must have a valid driver's license Must be able to take emergency call Lutheran Homes of South Carolina is an equal opportunity employer.
