Job Description

Elder Care Coordinator / Geriatric Care Manager Part-Time | $35-$40 per hour | Potential to Grow to Full-Time Our estate planning and elder law firm is expanding our services to include Life Care Planning, and we are looking for an experienced, compassionate, and highly organized Elder Care Coordinator / Geriatric Care Manager to help us build and grow this service. This is not a traditional law firm position. Our Elder Care Coordinator will work directly with older adults and their families to help them navigate the practical challenges that come with aging, including remaining safely at home, coordinating care, evaluating long-term care options, responding to changes in health or independence, and helping families know what to do next. You will work alongside our elder law and estate planning attorneys so that our clients' care, legal, and financial planning work together as one comprehensive plan. We are initially looking for someone part-time, with flexibility in scheduling depending upon the candidate and client needs. We expect the position to have the opportunity to grow into a full-time role as our Life Care Planning practice develops. Compensation is $35-$40 per hour, depending on experience and qualifications. What You'll Do As our Elder Care Coordinator, you will become a trusted point of contact for clients and their families.
Responsibilities will include:
Meeting with older adults and their families to assess current needs, concerns, goals, and available support Evaluating issues involving activities of daily living, safety, cognition, caregiver stress, housing, transportation, and other factors affecting a client's ability to remain independent Developing individualized Life Care Plans and practical recommendations for clients and their families Helping clients remain safely and independently at home when appropriate Identifying and coordinating home care, senior living, transportation, medical, social, and community resources Helping families understand the differences between independent living, assisted living, memory care, skilled nursing, home care, hospice, and other care options Assisting families with transitions between different levels of care Helping families evaluate care providers and senior living communities Communicating with family members and helping families work through difficult care decisions Following up with clients regularly and updating their Life Care Plans as circumstances change Maintaining organized documentation of client contacts, recommendations, action items, and follow-up needs Identifying changes that may require legal or financial planning and coordinating those issues with the firm's attorneys Developing relationships with high-quality senior care providers and community resources Serving as an advocate, resource, and steady point of contact for families navigating unfamiliar and stressful situations Who We're Looking For We are less concerned about a particular job title and more interested in finding the right person. Our ideal candidate has significant professional experience working directly with older adults and their families and understands how the senior-care system works in the real world. Strong candidates may come from backgrounds such as: Geriatric care management Social work Nursing Hospital or rehabilitation discharge planning Home health Hospice Assisted living or memory care Skilled nursing Aging services Senior living Other professional roles involving care coordination for older adults Prior experience working in a law firm is not required. We will teach the legal side of Life Care Planning and when an issue should be referred to one of our attorneys. The Person Who Will Thrive in This Position The right Elder Care Coordinator will be compassionate but decisive. You genuinely care about older adults and their families, but you are also comfortable making recommendations and helping overwhelmed families determine what needs to happen next. You should be an excellent communicator who can talk comfortably with an 85-year-old client, an overwhelmed adult child, a physician's office, a home-care agency, and an attorney. You should also be a practical problem solver. When a family calls because Dad is being discharged from the hospital tomorrow and Mom cannot safely care for him at home, your instinct should be to start identifying solutions. You must be able to remain calm under pressure. Our clients may be dealing with dementia, hospitalization, caregiver burnout, family disagreements, declining independence, or the possibility of nursing home placement. Strong organization is essential. You will manage multiple families at different stages, document your work, track follow-ups, and make sure important issues do not fall through the cracks. We are also looking for someone proactive who can identify problems, develop potential solutions, follow through, and keep the rest of the team informed without needing constant direction. You should be comfortable working collaboratively and understand that medical, care, financial, family, and legal issues often overlap. Qualifications We would ideally like candidates to have: At least 5 years of professional experience working with older adults, individuals with disabilities, caregivers, or families navigating long-term care Strong knowledge of senior-care resources and different levels of long-term care Experience assessing client needs and developing or coordinating care plans Excellent interpersonal and written communication skills Strong organizational and follow-through skills Comfort using technology and documenting client interactions The ability to work independently while collaborating closely with attorneys and other members of the firm A background or professional credential in social work, nursing, gerontology, care management, or a related field is strongly preferred. Experience with dementia care, geriatric care management, Medicaid, Medicare, veterans benefits, long-term care insurance, or other aging-related programs is a plus but is not required. What Makes This Opportunity Different Many professionals who work with older adults see the same problem repeatedly: families receive help with one piece of the puzzle, but no one is helping them put everything together. That is what we are trying to change. Our Life Care Planning practice will combine estate planning, elder law, long-term care planning, and ongoing care coordination so that families have a team they can turn to as circumstances change. This position offers an opportunity to build meaningful, long-term relationships with clients rather than simply helping them through a single hospitalization, placement, or episode of care. You will also have the opportunity to help us build the Life Care Planning program itself. We want someone who will bring ideas, experience, and knowledge of the local senior-care community to the role. As the program grows, we anticipate that this position may grow from part-time into a full-time role with additional responsibilities and opportunities.
