Role:
ComForCare Home Care - Clarksville is looking for a compassionate and organized Care Coordinator to join our growing team in Hendersonville, TN. In this role, you'll be the vital link between clients, caregivers, and families — ensuring every individual receives the highest quality of in-home care. If you're passionate about making a difference in people's lives, we want to hear from you! Must be willing to work part-time care coordinator 50% in the office and as a caregiver 50% in the field with our Clients.
Responsibilities:
Coordinate and schedule caregivers to match client needs, preferences, and care plans Serve as the primary point of contact for clients, families, and caregivers regarding care services Conduct client assessments and develop individualized care plans in collaboration with the care team Monitor caregiver performance and ensure compliance with home care standards Manage caregiver scheduling, shift changes, and on-call coverage to maintain continuity of care Maintain accurate client and caregiver records in compliance with regulatory requirements Assist with caregiver onboarding, training, and ongoing support Respond promptly to client concerns and resolve service issues with professionalism and empathy
Requirements:
Prior experience in home care, healthcare coordination, or a related field preferred Strong organizational and multitasking skills in a fast-paced environment Excellent communication and interpersonal skills with clients, families, and staff Proficiency with scheduling software and basic computer applications Ability to problem-solve quickly and remain calm under pressure Compassionate, patient-centered mindset with a commitment to quality care High school diploma or equivalent required; associate's or bachelor's degree in a health-related field a plus
