5/20/2026 About the Role Clarvida is seeking a Care Coordinator I to provide therapeutic services, case coordination, and resource linkage to individuals and families. In this role, you will work closely with clients and their support systems—including families, courts, and community partners—to promote behavioral change, stability, and overall well-being. This position is ideal for someone who is compassionate, organized, and comfortable working in community settings while collaborating with multidisciplinary teams.   • Conduct client intake, assessments, and community-based visits
  • Provide therapeutic interventions such as coping skills, behavior modification, parenting skills, and crisis intervention
  • Develop, implement, and update individualized treatment plans in collaboration with the care team
  • Coordinate services and act as a liaison between clients and external stakeholders (e.g., DCS, courts, community agencies)
  • Transport clients as needed and participate in on-call duties
  • Attend supervision and treatment team meetings, presenting client updates as required
  • Maintain accurate, timely, and compliant clinical documentation
  • Participate in peer audits and quality improvement initiatives
  • Support team collaboration to ensure positive client outcomes Requirements
  • Bachelor's degree in a Human Services field (e.g., social work, psychology, counseling)
  • Minimum of 1 year of experience in the mental health field
  • Knowledge of child development and behavior modification techniques
  • Strong communication, organizational, and teamwork skills
  • Cultural sensitivity and ability to work with diverse populations
  • Valid Driver's License and ability to travel (approximately 25%)
  • Ability to work flexible hours, including on-call responsibilities
  • Valid Driver's License and reliable transportation
  • Ability to pass all required background checks Preferred Qualifications
  • 1-3 years of experience working with children or adolescents in a therapeutic, community-based setting Work Environment & Physical Demands
  • Community-based role with moderate travel
  • Regular standing, walking, and sitting required
