Full-Time, Exempt Position Summary The Assessment Coordinator (QMHP-CS) is responsible for conducting comprehensive client assessments, developing and maintaining treatment plans, coordinating authorizations, and ensuring timely access to behavioral health services. This position serves as a key clinical support role within the organization and works collaboratively with the Community Health Worker (CHW) team, case management staff, and leadership to ensure high-quality, person-centered care. The Assessment Coordinator maintains a limited caseload while focusing primarily on assessments, treatment planning, service authorization, and clinical documentation. Essential Duties and Responsibilities Clinical Assessment and Treatment Planning Conduct behavioral health service-specific assessments in accordance with organizational policies and applicable regulations. Evaluate client needs and determine eligibility for programs and services. Develop, implement, review, and update individualized treatment plans. Complete reassessments and treatment plan reviews within required timelines. Ensure all assessments and treatment plans meet payer, regulatory, and accreditation standards. Coordinate referrals to appropriate internal and external service providers as needed. Authorizations and Documentation Prepare and submit prior authorization requests and supporting documentation. Monitor authorization approvals, renewals, and expiration dates. Maintain accurate, timely, and complete clinical documentation. Ensure client records comply with Medicaid, managed care, licensing, and organizational requirements. Assist with chart reviews and documentation compliance activities. Client Services Maintain a small caseload of assigned clients. Monitor client progress and collaborate with case management staff regarding service needs. Participate in care coordination meetings and interdisciplinary team discussions. Assist clients with access to appropriate behavioral health and community resources. Quality Assurance and Compliance Support quality improvement initiatives and compliance monitoring. Participate in internal audits and corrective action activities. Maintain compliance with HIPAA, confidentiality requirements, and all applicable federal and state regulations. Assist leadership with documentation reviews and program readiness activities. Collaboration and Support Provide consultation and guidance regarding assessments and treatment planning. Collaborate with CHWs, Case Managers, Supervisors, and Leadership to ensure continuity of care. Attend staff meetings, trainings, and clinical case reviews. Perform other duties as assigned consistent with the position. Qualifications Required Current QMHP-CS (Qualified Mental Health Professional-Child Specialist) credential in good standing. Bachelor's degree in Human Services, Psychology, Social Work, Counseling, or a related field. Knowledge of behavioral health assessments, treatment planning, and service authorization processes. Experience working with Medicaid and managed care requirements. Strong clinical documentation and organizational skills. Proficiency with electronic health records (EHRs) and Microsoft Office applications. Valid driver's license and reliable transportation. Preferred Experience in community-based behavioral health services. Experience with Virginia Medicaid services and prior authorizations. Experience conducting clinical assessments and developing treatment plans. Bilingual (English/Spanish) preferred. Knowledge, Skills, and Abilities Strong assessment and interviewing skills. Thorough understanding of trauma-informed and person-centered care. Ability to prioritize multiple deadlines and manage competing demands. Knowledge of behavioral health regulations and documentation requirements. Excellent interpersonal, written, and verbal communication skills. Ability to work independently and collaboratively within a multidisciplinary team. Physical Requirements Ability to sit, stand, walk, and use office equipment for extended periods. Ability to travel to program sites, community locations, and client appointments as necessary. Ability to lift and carry up to 20 pounds on an occasional basis. Work Environment This position may operate in office, clinic, community, and client home settings. Occasional evening or community-based work may be required based on client and organizational needs.

From $18.00 per hour Expected hours: 40.0 per week
