Job Description
Job Overview The Care Coordination Specialist at the SOAR Institute plays a critical role in ensuring that all patients, especially those who are uninsured and underinsured, receive timely and effective healthcare. We are seeking a dedicated and detail-oriented
BILINGUAL
Care Coordinator to join our team. In this role, you will be responsible for managing patient care processes, ensuring effective communication between patients and healthcare providers, and facilitating the delivery of high-quality medical services. The ideal candidate will have a strong background in medical terminology and administrative support. Join us in making a difference in patient care by applying your expertise as a Care Coordinator!Responsibilities:
Manage timelines and deadlines of each of your assigned patients including receiving patient consents, and all available medical records Effectively manage a case load of 150-300 cases Deliver patient case management service while maintaining a strong focus on patient medical treatment to ensure patients are compliant with plan prescribed by treating doctors, contacting patients on a regular basis to discuss care coordination updates, medical treatment status, and to answer questions. Take ownership over patient files for review and communication with referring accounts. Maintain accurate patient records and ensure compliance with privacy regulations. Communicate effectively with patients regarding their care plans, treatment options, and any necessary follow-up actions. Work consistently and proactively to address patient care issues; draft updates to referring and collaborating providers, and internal/external contacts regarding patient care Acquire, review and distribute medical and billing records throughout the treatment phase. Accurately maintain case management data in the case management system Identify issues and solve problems quickly. Communicate effectively with our internal billing and records specialists to ensure the correct and complete bills and medical records are obtained timely Enter detailed notes in EHR and keep cases updated daily Coordinate transportation for patients as required Answer all inbound communication and redirect as required. Assist healthcare providers with administrative tasks such as preparing documentation and managing office workflows. Collaborate with multidisciplinary teams to enhance patient outcomes and streamline care processes. Provide support in navigating insurance processes and addressing any billing inquiries from patients. Stay updated on medical terminology and office procedures to provide accurate information to patients and staff. Requirements Fluent in Spanish & English Bachelor's Degree preferred. Excellent time management, multi-tasking, and critical thinking abilities Salesforce experience a plus Positive attitude and a passion for helping people Professional appearance and behavior Experience maintaining a high degree of discretion dealing with confidential information Excellent customer service skills Strong written and oral communication skills Ability to work as a part of a team Excellent organization skills Punctual and Reliable Proficiency in Microsoft Office and ability to learn programs quickly Written and verbal fluency in both Spanish and English required Previous experience in a medical practice is a plus. Familiarity with medical terminology is essential for effective communication within the healthcare environment. Experience in providing medical administrative support, including scheduling, record management, and patient interaction. Strong organizational skills with the ability to manage multiple tasks efficiently while maintaining attention to detail. Excellent interpersonal skills to foster positive relationships with patients and healthcare professionals alike. #CAMP Job Type:
