This position serves as a central point of coordination between providers, field staff, virtual support staff, patients, caregivers, and other members of the care team. The ideal candidate is comfortable working in a fast-paced healthcare environment, managing multiple priorities, following established workflows, and ensuring that patient care tasks are completed accurately and on time. This role will have a strong focus on Chronic Care Management (CCM), Remote Patient Monitoring (RPM), patient scheduling oversight, clinical workflow coordination, documentation tracking, and office operations . Experience with Clinii and eClinicalWorks (eCW) is strongly preferred. Key Responsibilities Coordinate day-to-day office and clinical administrative operations for providers and field-based staff. Serve as a primary point of contact for providers, employees in the field, virtual assistants, patients, caregivers, and prospective patients. Support the organization's Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. Assist with enrollment, tracking, documentation, and ongoing coordination of patients participating in CCM and RPM services. Monitor CCM/RPM workflows to ensure required documentation, patient communication, and follow-up activities are completed. Work within Clinii to support RPM patient monitoring, device-related workflows, and patient follow-up. Utilize eClinicalWorks (eCW) for patient scheduling, documentation review, communication, and administrative coordination. Communicate with patients and caregivers regarding appointments, care coordination needs, and administrative questions. Assist with obtaining outside medical records, specialist documentation, hospital records, home health documentation, and other information needed for continuity of care. Coordinate communication between office staff and employees working in the field. Manage incoming phone calls, messages, and requests and route them appropriately. Maintain accurate spreadsheets, patient trackers, calendars, and internal workflow tools. Assist with monthly CCM/RPM reporting, documentation reconciliation, and preparation for billing workflows. Support patient consent tracking and other required documentation. Maintain confidentiality and follow HIPAA requirements when handling protected health information. Help identify workflow problems and communicate concerns to leadership before they become larger operational issues. Assist with general office operations and other administrative duties as assigned. Qualifications Previous experience in a medical office, healthcare coordination, care management, or clinical administrative support role preferred. Experience with Clinii strongly preferred. Experience with eClinicalWorks (eCW) strongly preferred. Familiarity with CCM, RPM, care coordination, field based primary care, or value-based healthcare workflows is highly desirable. Strong organizational and time-management skills. Ability to manage multiple employees, tasks, deadlines, and priorities simultaneously. Strong attention to detail and commitment to accurate documentation. Excellent verbal and written communication skills. Professional phone etiquette and strong patient/customer service skills. Ability to communicate effectively with providers, clinical staff, administrative staff, patients, and caregivers. Proficiency with Microsoft Office, Google Workspace, spreadsheets, and electronic documentation systems. Strong computer literacy and ability to learn new healthcare technology platforms quickly. Experience working with electronic health records (EHR/EMR) required; eCW experience preferred. Ability to work independently while also functioning effectively as part of a multidisciplinary team. Ability to recognize outstanding tasks and follow them through to completion without requiring constant supervision. Bilingual English/Spanish is a plus.
