Job Description

Job Overview We are seeking a compassionate and dedicated Hospice Licensed Master Social Worker (LMSW) to join our multidisciplinary team. The ideal candidate will provide comprehensive psychosocial support to patients and their families navigating end-of-life care within a hospice setting. This role involves conducting patient assessments, developing individualized care plans, and delivering clinical counseling to promote emotional well-being. The LMSW will collaborate with healthcare providers to ensure holistic, patient-centered care while adhering to HIPAA regulations and maintaining accurate medical documentation. Experience in hospice care, behavioral health, and working with diverse populations including children, seniors, and individuals with disabilities is highly valued. Responsibilities Conduct psychosocial assessments and diagnostic evaluations to determine patient needs and develop tailored intervention strategies. Provide individual, family, and group therapy utilizing modalities such as behavioral therapy, cognitive behavioral therapy, motivational interviewing, and clinical counseling. Support patients through crisis intervention, grief counseling, and mental health management related to terminal illness and palliative care. Collaborate with interdisciplinary teams to coordinate case management services, ensuring seamless integration of social work into overall patient care plans. Assist with intake processes, medical documentation, and maintaining accurate medical records within EMR systems while complying with HIPAA standards. Facilitate education on hospice services, advance directives, and community resources for patients and families. Engage in research activities related to hospice care practices or behavioral health interventions as needed. Work with diverse populations including those requiring Alzheimer's care, PTSD care, dementia care, autism spectrum support, or special education needs in outpatient or inpatient settings. Qualifications Master's degree in Social Work (MSW) from an accredited program; valid LMSW licensure required. Proven experience in hospice care or related fields such as mental health counseling, behavioral health, or medical social work. Familiarity with medical terminology, ICD coding (ICD-9/10), CPT coding, and documentation standards for healthcare settings. Knowledge of EMR systems and experience managing medical records securely under HIPAA regulations. Strong skills in crisis management, diagnostic evaluation, case management, and both individual/group psychotherapy. Experience working with children, seniors (geriatrics), individuals with developmental disabilities or disabilities requiring specialized support such as Alzheimer's or dementia care. Excellent communication skills with the ability to provide compassionate patient-centered counseling in high-pressure environments. Pay:
$60.00 - $90.00 per hour
