Care Coordinator - Behavioral Health Clinic (Full Time - Tyler) Are you passionate about helping others connect to the care and resources they need? Join our team at Andrews Center as a Care Coordinator and make a real difference every day! In this vital role, you'll work closely with a multidisciplinary team to identify client needs and link them to the right supports-whether that's within Andrews Center or through community resources. You'll help clients navigate referrals, coordinate aftercare with hospitals, and ensure smooth transitions so individuals can focus on what matters most: their wellness and recovery.
What We're Looking For:
