Job Summa This position is responsible for assisting with mental health care coordination, which can include connecting children and families with ongoing mental health needs to providers in the community for long-term care. Interest in developing sustainable pathways for referrals and enhancing processes is welcomed. This position provides administrative support and coordination for the Lake Street Clinic to maintain a high level of efficiency. The position requires the ability to function effectively as a team member while also having the ability to work independently with moderate supervision Major Responsibilities/Duties/Critical Tasks Work in an inter-professional model and collaborate with pediatric psychiatrists, pediatric psychologists, and administrative staff to identify the long-term psychological needs patients. Assemble information concerning patients' clinical background and referral needs. Connect patients and families to community resources for on-going care or follow up, providing ongoing support and facilitation to ensure effective transition. Coordinate with Lake Street Clinic professionals, schools, and families to minimize obstacles and increase patient and provider satisfaction. Assist patient in completion of forms/applications required to receive needed services or as required by referral agencies. Maintain confidentiality of information as appropriate in all aspects of care. Support team members in disseminating appropriate referral information. Inputting patient data (name, date of birth, insuranceâEUR¦) into EMR Demonstrates professional behavior in all interactions and utilizes proper telephone etiquette during every phone call. Adheres to internal controls established for the department. Other duties as assigned Required Certifications/Licenses Bachelor's degree in Social Work, Psychology, or related field; May accept a combination of education and experience. Excellent written and verbal communication. Demonstrated knowledge working with deadlines and under pressure and the subsequent ability to prioritize tasks. Ability to be flexible, make sound decisions and demonstrate problem-solving skills. Accompanying Knowledge, Skills, Abilities and Competencies Demonstrates professional behavior with sound work habits. Customer service skills. Ability to multi-task in a fast-paced working environment. Knowledge of managed care, medical terminology, patient referral process, and claims Experience working with pediatric behavioral and mental health patient populations.
