In this vital role, you will serve as a key point of contact for patients and their families during weekend hours, ensuring seamless coordination of care across various medical disciplines. Coordinates care through assessments, training, caregiver support, and communication with clients and families., all while maintaining a compassionate and patient-centered environment.
