We are looking for a Resident Care Coordinator to join our team and help provide care to our Assisted Living Facility. The successful candidate will be responsible for coordinating patient care between medical providers and ensuring that all services are provided in a timely manner and according to the patient's needs. The Care Coordinator is a professional Care Team Member trained to provide and oversea personal care and related services to residents in the community according to documented Care Plans (CP). The RCC also assists in the training, supervision, and motivating the Medication Technicians and Caregivers.

Responsibilities:

• The RCC is responsible for assisting with the complete implementation of the CP specific to each resident ensuring that person centered care is explained in a way that fosters consistent approach and results.
• The RCC ensure that any additional communication through Policy and Procedures such as bath lists, laundry lists, daily task list, etc. reflect each resident's person-centered needs.
• The RCC monitors the task completion in electronic system and monitors for changes in resident needs.
• Maintain accurate records of client progress and services provided.
• Provide crisis intervention services when needed.
• Monitor client compliance with treatment plans.
• Communicates with the Wellness Director about changes in the resident's status.
• Assists the resident in maintaining a safe and clean environment.
• The RCC updates resident care plans as requested.
• This position requires on call for staffing, availability on the weekends, and availability on holidays.
