LMSW Case Manager in
Healthcare & Medical Services Responsibilities:
Assess psychosocial needs of patients Develop person-centered care plans Coordinate services and advocate for patients Collaborate with physicians, nurses, and community partners Support safe discharges, reduce readmissions, and improve health outcomes Utilize data, EMR tools, and evidence-based practices to enhance workflows and quality metrics Contribute to improving patient experience and access to care
Qualifications & Skills:
Licensed Master Social Worker (LMSW) license Strong assessment and care planning abilities Excellent collaboration and communication skills Ability to utilize data and EMR tools effectively Commitment to patient-centered, innovative, and evidence-based practice
