Responsibilities include:
Meet with clients to assess and coordinate their care needs Advocate on the client's behalf with facilities, physicians and family members Attend appointments, schedule meetings and manage care plans for clients Coordinate and collaborate with trusted community partners This is an hourly position with generous compensation and a 401k. In this position you will set your own schedule and hours, working full time, around 35 hours a week. One call weekend coverage approximately once per quarter with bonus and time and a half hourly compensation. Candidates must have strong interpersonal skills, excellent oral and written communication skills, possess strong organizational and problem-solving skills, good record keeping, and the ability to set priorities and meet goals. CURRENTLY RESIDE IN NORTHERN
VIRGINIA, specifically eastern Fairfax County, Arlington County, or the City of Alexandria, will be considered. Applicants must hold a Master's degree or professional degree with appropriate licensure as a Registered Nurse (RN), Occupational Therapist (OT), or Licensed Social Worker (LSW/LCSW); relevant professional certification is a plus. One to three years of relevant experience is required, with experience in geriatric care management, health care, medical social work, or case management strongly preferred. Candidates must have access to a computer and the ability to maintain electronic records, along with reliable use of a personal vehicle for client visits. Appropriate state licensure and valid auto insurance are required.
