at Northeast Kingdom Human Services in Newport, Vermont, United States Job Description Care Coordinator Join our team in a pivotal role where you'll use a person-centered approach to coordinate care across various health services. As a Care Coordinator you'll be instrumental in providing access to physical and behavioral health care, social services, housing, education, and employment opportunities, all aimed at fostering wellness and supporting the recovery of the whole person. Your expertise in care coordination will ensure alignment with the preferences and needs of those you serve, staying in line with their current treatment plans. RESPONSIBILITIES & SCOPE
Promote community integration of individuals and assist/encourage natural supports necessary. Counsel clients about the use of the National Suicide and Crisis Lifeline, 988, local hotlines, warm lines, mobile crisis, and other needed resources. Assess clients need/desire in accessing benefits, such as Medicaid and financial assistance. Develop partnerships with a variety of community services and supports. Contact any clients that have been discharged from an inpatient setting within 24 hours of discharge to connect them to services. Provide service coordination with a medical focus among internal and external providers. Reviews schedule daily and contacts clients that no show and reschedule immediately.
