The Clinic Care Coordinator serves as a key point of contact for patients receiving care at Rutland County Health Partners Free Care Clinic. This position provides patient support, care coordination, health education, and navigation assistance to help individuals effectively access and navigate healthcare services. The Clinic Care Coordinator works particularly with underserved and uninsured populations and collaborates with healthcare providers, community organizations, and other resources to support continuity of care.
ESSENTIAL JOB RESPONSIBILITIES
Patient Navigation & Care Coordination Guide patients through the healthcare system, including referrals, appointments, follow-up care, and access to appropriate healthcare and community services. Coordinate care with external healthcare providers and community resources, including, but not limited to, the YouFirst and Wise Women programs. Provide insurance navigation support, including assistance with enrollment through Vermont Health Connect. Provide tobacco cessation support, including completion of required training and administration of Nicotine Replacement Therapy as permitted by applicable scope of practice and clinic protocols. Complete additional patient navigation activities as needed to support grant objectives, program goals, and reporting requirements. Clinical Support Provide clinical and patient support within the scope of training, certification, and licensure. Assist providers with patient care, examinations, procedures, and routine clinical tasks. Obtain and document patient information, vital signs, and other appropriate clinical data. Administer vaccines and medications as permitted by applicable scope of practice, clinic protocols, and Vermont Department of Health (VDH) requirements. Prepare examination rooms, equipment, and supplies and maintain a clean, safe, and organized clinical environment. Support patient education, care coordination, and follow-up as directed. Community Outreach Connect patients with social services, community resources, and other support programs. Participate in community outreach activities and events, including events involving the Mobile Health Unit. Represent the Clinic in a professional, respectful, and patient-centered manner at community events and partner activities.
OTHER RESPONSIBILITIES
Administrative & Program Support Assist the Executive Director, Medical Director, Dental Director, and other staff in monitoring and maintaining the Clinic's Quality Improvement Program. Monitor clinical policies and procedures and assist with updates, implementation, and education for clinical volunteers as appropriate. Assist with data collection, documentation, and reporting related to clinic programs, grants, and quality improvement activities. Attend staff meetings and participate in program planning and development discussions. Support special projects and other administrative duties as needed. Contribute to an inclusive, supportive, and respectful environment for patients, staff, volunteers, and community members.
EDUCATION & EXPERIENCE
Required Current certification or licensure as a Registered Nurse (RN), Licensed Practical Nurse (LPN), Medical Assistant (MA), or Licensed Nursing Assistant (LNA) . Minimum of two years of clinical experience. Current CPR certification. Excellent written and verbal communication skills. Strong organizational skills and attention to detail. Ability to work independently and effectively as part of a small, dedicated team. Preferred Experience in care navigation, care coordination, community health, or a related setting. Familiarity with the needs and resources of underserved populations in rural Vermont. Proficiency with Microsoft Office, social media platforms, and basic website management.
WORK CONDITIONS
Full-time or part-time position, up to 32 hours per week. Flexible schedule, with some evening and weekend hours required. Approximately four hours per month supporting the Mobile Health Unit at community screening and outreach events. Valid driver's license and reliable transportation required. Work is performed primarily in person at the Clinic, with community-based work as needed.
COMPENSATION & BENEFITS
